| Federal Reserve Bank External Subscriber Request Form An organization's EUAC should complete a separate Federal Reserve Bank External Subscriber Request Form for each individual who will be authorized to access a FedLine access solution. This online, interactive form authorizes the Federal Reserve Banks to create a credential granting the Subscriber access to the Federal Reserve Financial Services indicated on the form. |

| Federal Reserve Bank End User Authorization Contact (EUAC) Form for FedLine Web® and FedLine Advantage® This form gives the EUAC authority to designate authorized individuals (Subscribers) who should be issued credentials to enable them to transact business using a FedLine access solution. When we receive the completed End User Authorization Contact Form, a credential will be issued to the EUAC. The EUAC will use the credential to access the EUAC Center, which enables the EUAC to reset FedLine subscriber passwords and access Subscriber reports and FedLine documentation. NOTE: This form must be signed by an individual listed on your organization's Official Authorization List (OAL). |
